Terms and Conditions for Participation in the Wellness Coach Beta Program

1. Acceptance of Terms

By participating in the Wellness Coach Beta Program and submitting feedback through this form, you agree to the following terms and conditions.

2. Eligibility

Participation in the Beta Program is voluntary and open to users who have received an invitation from Wellness Coach. You must be at least 18 years old to participate.

3. Confidentiality

As a beta tester, you may have access to confidential and proprietary information about Wellness Coach, including unreleased features and updates. You agree not to disclose, share, or publish any information regarding the Beta Program or your participation without prior written consent from Wellness Coach.

4. Feedback

All feedback, suggestions, ideas, or reports you provide through this form or other channels during the Beta Program become the property of Wellness Coach. Wellness Coach may use, modify, or distribute this feedback for any purpose without compensation or acknowledgment.

5. Reward

Participants who submit feedback via this form will be eligible to receive a $10 reward, provided that the feedback is complete and relevant to the Beta Program. The reward will be issued via the method specified by Wellness Coach within 30 days of feedback submission.

6. No Warranty

The beta version of the Wellness Coach app is provided on an "as-is" basis. Wellness Coach does not guarantee that the app will be free of defects, errors, or bugs. Your participation in the Beta Program is at your own risk.

7. Liability

Wellness Coach will not be liable for any direct, indirect, incidental, or consequential damages arising from your participation in the Beta Program, including but not limited to loss of data, system failure, or any other damages related to the use of the beta version of the app.

8. Termination

Wellness Coach reserves the right to terminate your participation in the Beta Program at any time and for any reason, without prior notice.

9. Modification of Terms

Wellness Coach may modify these terms and conditions at any time. Continued participation in the Beta Program after any such modification constitutes your agreement to the new terms.

10. Governing Law

These terms and conditions are governed by and construed in accordance with the laws of [Your Jurisdiction], without regard to its conflict of law principles.
